Below are mandatory forms you are required to review and complete prior to being seen by in our office. Virtually every medical practice, by law, is required to provide these essential documents. When you call and make you an appointment, you will be sent a link (via email or texted to your cell phone). If you are unable to do this electronically, please download, review and complete each of the three forms below prior to your appointment.
Patient Financial Responsibilities. Because you are ultimately responsible for the costs associated with your care, this document elaborates your financial responsibilities.
Notice of Privacy Practices. We are required to maintain the privacy of the health information you provide to us. This document describes how medical information may be used for your benefit, in order to optimize your health care.
Patient Intake (Medical History) If you are new to our practice or are an established patient who has not seen us for over a year, please download, print and complete the this form, and bring it with you on the day of your appointment. Believe it or not, the information you provide on this form is extremely helpful in optimizing your care, and will help expedite your seeing the provider in the most timely fashion.
